Description (partial)

Symptom:
Supervisor or xbar modules are reloaded when errors on the inband links to the fabric modules reaches a preset threshold. The affected module may not come back online due to the errors being persistent.

If the affected supervisor is a standby supervisor, it may be put into 'powered-down' state. The following syslog messages may be logged:

  %DAEMON-2-SYSTEM_MSG: <<%XBAR-2-XBM_EEM_MODULE_ACTION>> Module slot 8 has been reloaded by xbar-manager because of an eem-policy triggered action xbm_eem_err_mgmt_fn 597  - xbar[2806]
  %MODULE-2-MOD_DIAG_FAIL: Module 8 (serial: JAB1039030E) reported failure on ports 8/1-8/1 (Supervisor Inband) due to Module asic(s) reported sync loss (DevErr is LinkNum). Trying to Resync in device 88 (device error 0x0)
  %SYSMGR-2-STANDBY_BOOT_FAILED: Standby supervisor failed to boot up.

The standby supervisor reset reason may show as follows:

    Reason: Reset of standby by active sup due to sysmgr timeout


If the affected supervisor is the active supervisor a supervisor switchover will occur. The following supervisor reset reason is logged:

      Reason: Reset Requested due to Fatal System Error

If both supervisors are affected the system will reboot.

If the affected module is an xbar module the following syslog message may be logged (slot 14 = xbar 1, slot 15 = xbar 2):

  %DAEMON-2-SYSTEM_MSG: <<%XBAR-2-XBM_EEM_FABRIC_ACTION>> Fabric slot 14 has been reloaded  by xbar-manager because of an eem-policy triggered action xbm_eem_err_mgmt_fn 581  - xbar[2806]
  %DAEMON-2-SYSTEM_MSG: <<%XBAR-2-XBM_EEM_FABRIC_ACTION>> Fabric slot 15 has been reloaded  by xbar-manager because of an eem-policy triggered action xbm_eem_err_mgmt_fn 581  - xbar[2806]

Conditions:
This issue only occurs on Cisco MDS 9513 chassis with DS-13SLT-FAB3 fabric modules.
