6901, 6911, 6921, 6941, 6945, or 6961 phones that are put on hold and should be hearing music on hold instead hear silence.  This will happen intermittently and not every time.

6900 or 69XX series phones are configured to receive multicast music on hold (MoH).  The phone intermittently fails to send an IGMPv2 membership report for the multicast stream so the phone will not join ask to join the multicast stream and thus will not receive the multicast stream.  The phone should be sending an IGMPv2 membership report sourced from the IP address of the phone destined to the multicast IP address it wants to join.
