Symptom:
PS-FAN falls to FAUTY status after upgrading your IOS software.

The show env  stack command displays the following

SWITCH: 1
FAN 1 is OK
FAN 2 is OK
PS-FAN1 is FAULTY
PS-FAN2 is NOT PRESENT
TEMPERATURE is OK
Temperature Value: 35 Degree Celsius
Temperature State: GREEN
Yellow Threshold : 46 Degree Celsius
Red Threshold    : 60 Degree Celsius
POWER is OK
RPS is NOT PRESENT

SWITCH: 2
FAN 1 is OK
FAN 2 is OK
PS-FAN1 is FAULTY
PS-FAN2 is NOT PRESENT
TEMPERATURE is OK
Temperature Value: 34 Degree Celsius
Temperature State: GREEN
Yellow Threshold : 46 Degree Celsius
Red Threshold    : 60 Degree Celsius
POWER is OK
RPS is NOT PRESENT

PS-FAN falls to FAUTY status after upgrading IOS. <key>CSCuo97298</key>   I found this issue to exist on version 15.2(2)E as well, I downgraded to 15.2(1)E3 and the problem went away.
