Description (partial)

Symptom:
After a Supervisor Switchover (SSO) event, an IPC port open failure occurs and all cards (TCCs and DS48-1G linecards) restart due to heartbeat timeouts.  Example:

Following Error Logged for All Cards:
%RFGW-6-HEARTBEAT_WARNING: Heartbeat process has failed to receive heartbeats (for 3 consecutive seconds), for linecard (X)

IPC Port Open Failure Logged:
%RFGW-3-IPC_PORT_OPEN: IPC port open RFGW: lc (X):tcc front panel failed for reason

%RFGW-5-TCC_CARD_RESET: TCC Module (X) has been reset with no available standby; resetting

Each Module Goes Offline/Removed

Conditions:
This occurs after a Supervisor Switchover (SSO)
