Symptom:
The following behavior after adding Directory Number to the phone configuration has to be documented  in this document under "Association Information " on this guide :

The note should read as  follows :-

" On this callmanger version the directory number after being added to a phone  has to  be saved  twice on the callmanger  page also  the following message would be  seen  after saving  the DN configuration the first time - 
Directory Number Configuration has refreshed due to a directory number change. Please click Save button to save the configuration."

following callmanger versions :

10.0.1.10000-24

10.0.1.10000-22

9.1.2.10000-28
