Description (partial)

Symptom:
After a switch reboot (power off then on), switch sends a Failure packet to the PCs (Win7 and WinXP) and causes PCs to fail dot1x authentication.

Conditions:
Issue was observed on the following platforms:

3750, 3560, and 2960 both running 12.2(55)SE64
510R+E with Sup 7E running 15.1(2)SG1
6509-E with Sup720 running 122-33.SXJ6
