Cisco Bug: CSCuo19684 - WLC 5508 fails to load the primary image after successful image download

Description (partial)

Symptom:
The controller is running 7.4.100.60 and customer did an upgrade to 7.4.121.0, the upgrade was successful however after reloading the controller, the controller always goes back to 7.4.100.60

Even tried to force the boot from 7.4.121.0 however the controller still loads with 7.4.100.60, tried to reload the image(7.4.121.0) once more however we are still noticing the issue.

From the logs we notice the below details :-
