Description (partial)

In most failures, the card simply stops responding and times out abruptly or does not show up in "show inv" command. All LEDs are off and reseating the card does not help. Occasionally, the card may get stuck in a reboot loop and doesn't come up successfully

This failure is due to the chip internal design defect, after the long time work, the voltage reference inside the chip outputs voltage would increase for Hydrogen Compensation, then this voltage goes too high, it would destroy the MOS FET inside the chip and make the failure, the Hydrogen Compensation effect is related to chip working time, the failure rate would increase as working time increasing.
