Description (partial)

- Set / Remove CFA from IP Phone sofkey, CDR record will be inserted with Forward / Simple types respectively.
- Set / Remote CFA from ccmuser, ccmadmin or CTI/AXL application (i.e. Jabber), no records will show up.

CDR Log Calls with Zero Duration Flag set to True.
