Description (partial)

When trying to self-provision a new device for a user using the IVR method, the system plays a message saying "This device could not be associated to your account.  Please contact the system administrator to complete provisioning."

This issue occurs when the End User has"Use System Default" set for the user profile.  This occurs even though the System Default user profile has the correct configuration.
