Description (partial)

Symptom:
Calls using H323 ICT fail after a mid-call codec change with cause code 47 due to Media Exchange Timeout.  We see the incoming H.245 MasterSlaveDetermination gets discarded due to the call state not being changed correctly.  CCM traces should show "ProcessH245SM - Unexpected MSD request received. Discarded".

Conditions:
Using H323 ICT with the codec changing after the initial call setup resulting in H.245 setup starting over.
