The startup configuration can be cleared/wiped unexpectedly though a few methods, most notably frequent loss of power.

Doing the power cycle multiple times randomly before launching the IOS console, startup config is getting cleared from NVRAM. It doesn't happen every time, since it's based on the timing of the power loss.
