A Cisco UCS blade fitted with a Cisco Virtual Interface Card (VIC) can encounter a condition that results in repeated terminate failures.

This condition has been seen on environments where the storage encounters a problem that results in termination of connections from the host. This is typically via LOGOs sent to those host.
