Symptom:
The following DSP errors are seen once the condition has been triggered:

DSP2 flex_vic_config_ack Failed. ret code = 0x0080

flex_set_codec_state_send_with_ack: Timeout condition! ack_status = 11, time_passed = 1501, channel_id = 1

Conditions:
After enabling cable-detect on a loopstart FXO port, saving the routers config, and reloading the GW upon returning from reload  IOS polls the DSPs to quickly and causes DSP firmware download failures.  The issue occurs on IOS voice gateways running 151-3.T2 through 151-3.T4 and 151-4.M1 through 151-4.M7 using PVDM2 DSP modules and FXO ports.
