Description (partial)

Symptom:
Rare unexpected MIO switchiover followed by chassis reload during a processing of DPC card crash.

Conditions:
Critical task blocking timeout leads to MIO switchover when processing a card state change event due to a DPC going down. 

HAT timeout of CSP blocking message to afctrl led to MIO6 switchover to MIO5.   
Upon becoming active MIO5 attempted to warm start it's FSC fabric interface drivers.
MIO5 could not read setup information from SCT database so warm start aborted.
MIO5 failure to warm start fabric interfaces requires cold reset of FSC to recover.
MIO5 cold reset of FSC leads to control plane loss and reload of chassis since MIO6 is still unavailable
