Description (partial)

Symptom:
A Nexus 5000 switch reloads after gatosusd process crashes.

`show system reset-reason`

    Reason: Reset triggered due to HA policy of Reset
    Service: gatosusd hap reset
<snip>

Conditions:
Following log messages can be seen in the syslog file:

- KERN 2 SYSTEM_MSG usd process 3372, uuid 657 (0x291) failed to send heartbeat - kernel * 
- SYSMGR 2 SERVICE_CRASHED Service "gatosusd" (PID 3372) hasn''t caught signal 6 (core will be saved). * 
- SYSMGR 2 HAP_FAILURE_SUP_RESET System reset due to service "gatosusd" in vdc 1 has had a hap failure *

And the following messages can be seen in the output of `show processes log details` command:

- Invalid Desc Fetch Index detected on Channel 0, asic 4
- ERR:  Disabling STM update DMA channel on gatos 4 due to error
