Description (partial)

Symptom:
User Options pages are not populating all the fields when accessing them through a Chrome Browser. All that is seen is the phone and line but none of the provisioning options are available. Rendering the page unusable for information or configuration.

Conditions:
When trying to access the User Options pages in 9.x using a Chrome browser.
