Description (partial)

Symptom:
Issue 1:
Power supplies goes into unpowered state and in the log "Input Or-ing B Fault" alarm is observed randomly. Red LED starts glowing. 

Issue 2:
Intermitent fan failure logs are seen
%PLATFORM-PWR_MGMT-2-MODULE_FAILURE : Power-module 0/PM0/SP failure condition raised : Module fan failure observed

Conditions:
Issues may be seen during initial power module insertion, or during normal operation.

Asr9k Power Supply issue bug <key>CSCuj40787</key>
I have this issue on one of my asr9k routers.  pmo4 &amp; pmo12.  re-seating these made no difference.  so swapped pmo4 with pmo0 and pmo12 with pmo8.  I am now seeing alarms for pmo0 and pmo8 should I be looking to replace these pmo's ?
