Description (partial)

Symptom:
After a QoS policy is removed, show running-config shows it's removed but show policy-map targets still shows it's there. If user configures a policy on the same interface in the same direction, commit shows no error but policy is not applied. The interface is stuck in this QoS inconsistency state.

Conditions:
Either of the following happens
1. A rare double failure: In place policy modification fails when applying the new policy, when rolling back to the original policy, some how it fails (usually doesn't) again, it will trigger this bug.

2. QoS related process restart due to SMU installation or crash. Normally the HA feature will restore previous configuration and should match the config committed in database. If for some reason there is internal data corruption/inconsistency so that the replayed qos config from committed database is not configured in hardware, it will trigger this bug.
