Description (partial)

Symptom:
Symptoms may include one or more of the following:

1.  CUCM does not send a 200 OK to a BYE received to disconnect a call
2.  Directory numbers on CTI Ports or phones may be busy unexpectedly until they are reset
3.  A memory leak in the ccm process may occur.

Conditions:
This is an intermittent race condition in the way that the CAC layer within the ccm process.  There is no known trigger or way to force this to occur.
