Description (partial)

Symptom:
Broadcast reply packet to a relay agent will be dropped if the giaddr does not match the relay agent ip address.  Even if the giaddr or relay address is 0.0.0.0.

Conditions:
This can happen when the broadcast flag is set and reply(offer and ack) packets from server only.
The first relay agent will flood the packet correctly as the giaddr is its own.
The packet will be dropped when received by the second relay agent.
