Symptom: After multiple "hw-module location <fabric card> reload", fabric card will end up with FAILED state with a small probility. Through the console, you will see the message below. "0/RP0:Jan 28 07:41:49.719 : shelf_mgr: %INFRA-SHELF_MGR-6-HW_EVENT : Rcvd HW event HW_EVENT_FAILURE, event_reason_str 'pon exit <-- BOARD_UP cnt=0! exit_code=undefined' for card 0/FC4 0/RP0:Jan 28 07:41:49.722 : shelf_mgr: %INFRA-SHELF_MGR-3-CARD_SW_FAILED : Card: 0/FC4 software state going to FAILED 0/RP0:Jan 28 07:41:49.725 : shelf_mgr: %INFRA-SHELF_MGR-3-CARD_HW_FAILED : Card: 0/FC4 hardware state going to FAILED " Conditions: This issue will happen with very small probability after reloading a fabric card for multiple times.
