Description (partial)

Symptom:
Order failed for voicemail template

Conditions:
Step 1:
Login to UPM and navigate to "Design -> User provisioning setup"
Step 2:
Create a new service template for voicemail service.
Step 3: 
Add the service area and user role
Fill the necessary provisioning attributes and save the template.
Step 4:
Navigate to "Design-> User Provisioning" and create a new subscriber for the same domain and associated the same user role.
Step 5:
Click the "New service" and select the service area.
Step 6:
Place an order for line and then select the voicemail service from the list.
Step 7:
Using the created voicemail template, place an order.
Final, it is showing a unrecoverable error message for that voice mail order
"Status:   RELEASED -  Unrecoverable Error 
IPT-0600: Software - Failed to create subscriber User_130910153941 | Cause - org.apache.axis2.AxisFault: com.cisco.unity.cuals.CuDbException: Missing key in referenced table for referential constraint (informix.fk_tbl_callaction_targetconversation_tbl_conversation_conver)."
