Description (partial)

Symptom:
I you plug the USB headset to the MacBook (or mac with built in microphone) and then unplug it, intermittent audio xmit failures were observed.  The other side couldn't hear the audio.  The user who unplugged the headset doesn't know if his audio is being sent or not.

Conditions:
This is intermittent problem so the problem isn't always observed for given condition.  Given that, the problem was observed during the J4M call only after the user plugged and then unplugged the USB headset.  Also, the problem seems to happen more frequently with Plantronic C420 then other USB headset.
