Description (partial)

Symptom:

The GTK key doesn't use key index 1 & 2 but always index 0. 
This behavior caused inter-operability problem and some clients
(Windows Broadcom  driver) get disconnected from AP.

Conditions:

When using dynamic WEP, when the broadcast key rotation timer expires, index 0
is used.
