Description (partial)

Symptom:
-         OS Password Reset fails with "pwrecovery" process.

Conditions:
-         Login as "pwrecovery" and password "pwreset"
-         It will ask you to remove any drives from your DVD.
-         Remove a drive and then hit any key to continue.
-         It will then prompt you to Insert any valid CD.
-         I insert a Valid CD and hit any key.
-         Then it prompts you with options to set OS Password.
-         You select the OS Password Option and set the passwords.
-         Then you try to login again and it never allows you to and results in login failure.
