ACS 4.2.0 code can encounter conditions in which the csauth process stalls, being unable to allocate
further threads.  Behavior observed is the device will stop authenticating users entirely until either
csauth is restarted or all services are restarted on the server.


Issue is observed when multiple authentication or accounting requests are received for the same
username from the same NAS and NAS port.

Within a working environment these conditions appear to occur under extremely bursty traffic loads,
although this likely doesn't directly affect results
