Outlook scheduling fails with "Participant already added to meeting"

User set up a large meeting (>30 participants) for an Executive Review and received an error each time he sent the meeting - something like "Meeting cannot be scheduled - participant already scheduled in this meeting".  He tried resending and still no success, same result.  So then he went to web server user interface and saw 5 meetings with same title but different IDs.  But no meeting exists on his outlook calendar and he cannot edit or delete the meeting from the web interface.  He cannot determine if meeting invites were sent or if he needed to resend the meeeting invite.  And if he did a resend he was not sure if it would work.
