Description (partial)

Symptom:
The service QoS may not be what was specified by the PCRF.

Conditions:
This can happen if the following sequence of events occurs:
1) PCRF sends a Service-Status AVP with Cisco-QoS AVP.
2) PCRF updated the Service-Status AVP without Cisco-QoS AVP.
3) CSG2 creates a service instance with the same name as the
    Service Name specified in the Service-Status AVP.
