Description (partial)

Symptom:
In SkillGroupHalfHour, only RouterCallsOffered field is counted for the call.
In CallTypeHalfHour, CallsRequeried is counted. In RCD, RequeryResult is 4.
In Windows Event Viewer, router generated error code 495.

Conditions:
It happens when all of the following conditions exist:
1.  QToSG node is used with requery enabled, 
2. The targeted agent does not have ATR rule or device target label configured
3. No default label is configured for the dialed number
