Description (partial)

Symptom:
During 3-way call, audio of 1st leg is heard while TWC initiator is waiting for 2nd party to answer.

Conditions:
Scenario:

A--calls--> B (A+B in call)
A--puts B on hold (MDCX "inactive")
A--dials C having TAT,
Call Failed on AS (504 message returned)
A gets connected to C directly w/o A hook-flash.

Problem : 'A' can hear both B & C legs before A hook-flash to connect B+C.
