Description (partial)

When using Phone Proxy feature on Adaptive Security Appliance (ASA) to call between an outside phone to Public Switched Telephone Network (PSTN) phone behind a gateway on the inside, muting the PSTN phone for an extended period of time may result in blocked voice traffic between the two endpoints. Unmuting the PSTN phone may not resolve the problem after a longer period of time.

Voice Activity Detection (VAD) enabled on the PSTN gateway and no audio coming back from the PSTN phone.
