Description (partial)

Symptom:

When a SIP SRST phone transfers a call to another SIP SRST phone,
the call can get stuck in a held state if the transfer-to party does 
not answer.

Conditions:

If A calls B and B answers and then B transfers the call to C, if
C does not answer before A hangs up, the call could get stuck
in a held state which cannot be cleared by the user for a 
minute or two.
