Description (partial)

When a device is placed in "Repair Mode" due to poor performance, device errors, or device resets, the evaluators are paused.  The paused evaluators prevent the storage system from completing its load-balancing or resiliency tasks.  This bug prevents a device from leaving the repair mode.  Although the system may appear to remain operational it will not achieve the expected level of performance or resiliency.  

A device in this state will post repeated messages to /var/log/debugmessages similar to the following:
"cdd:err: csd10: IO error 0x2 - ok : ok : CHECK_CONDITION".  
This condition is the result of a rare device failure mode wherein the device does not return error context with an initial failure event.  The missing context renders the driver unable to accurately interpret the error condition and the device remains in repair mode.
