Symptom: When trying to add a user on a clustered system, it fails with the error message: Duplicate mailbox id. A mailbox with that id already exists. Unable to create User even though the user has never existed on the system. Conditions: This can occur in a Cisco Unity Connection 7.x active/active cluster.
