Description (partial)

Symptom:
First the card fails to get activated with the following error message, 
"%MCX-3-INITFAIL: MCX (bay 1), Init Failed .." due to hardware issue.

The system will crash on further configuring the card type command.

Conditions:
The issue is seen in 7600 series router with PA-MC(X)-(2,4,8)TE1 family of port adaptors.
