Description (partial)

Duplicate emails are seen when a single email is sent. It could be a couple
duplicated emails received or the same email that keeps popping up.

Outlook 2003 mails bounced back with "client operation failed" error in rare
Conditions that would cause a TCP connection that is part of the Outlook
client application to reach the Exchange server directly (bypassing the WAEs):
 (1) WCCP ACL's at the router (br and/or core) to deny redirect
 (2) Network level configuration changes (interface up/down, TFO optimization
enable/disable, etc.) causing transient conditions that can leak connections
from the client to the server.
In addition to this, TFO Overload conditions at the branch and/or core
coinciding with heavy usage of the Outlook (causing several concurrent TCP
connections from the same Outlook session).
