Description (partial)

Symptom:
Wideband Cable Modems are w-online but cannot be pinged.
DOCSIS pings to CMs can be done successfully.
This might result in loss of IP connectivity for the modems.
1000 pings to the modem recovers the modem.

Conditions:
This problem has been observed in BC3. It could also happen in other releases
like Amazon or cobra_cable since the fix does not exist in those branches either.

When the CMTS has been running for a long time, it uses a previously-used DSID for a CM. 
This may result in using a non-zero value as a packet sequence number in the DS EHDR of the first packet.
